This week's topic is pretty clear to anybody that's ever been to a movie and felt like they didn't understand the plot, went to a restaurant and felt a bit disappointed, or read a book and became an instant fan of the author. Surprise! Truths and myths are a problem in medicine. Pain medicine is unique to the desperation that people have to escape its grip. People will do anything, take anything, try anything, and buy anything to feel better. Pain is a demon, and there are plenty of people that will tell you that they have the solution to your desperation. In this episode we're going to talk a bit about spine pain, medications, controlled substances, noncontrolled substances, and introduce the listener to this entire concept. Throughout this podcast series we will be talking about what has evidence based support, and what is just junk. Leave some feedback. Why, How, Who. Epidemiology? SI, Differential what?

This sounds like a good bedtime story. Actually it's pretty interesting. As we gather an understanding of a particular problem in medicine, we want to see where it goes, what happens to it, who gets it and who develops this problem and why they develop this problem. We're putting the building blocks down of understanding how to approach a patient in pain. Actually, any patient. It's called the natural history. Not particularly cool sounding, but is a very important fundamental to understand where you're going to be not now, but in 3, 6, 9 and 12 months. Also, woven into this is the differential diagnosis. The differential diagnosis of a disease state is "what it can be." Based on physical findings, historical elements, and just plain common sense and experience can be the key to unlocking a medical problem, and defining Rule 2. Finally, the sacroiliac joint is one of the most common pain generators and often completely misunderstood. We're touching on this, it's a start of a discussion about sacral joint pain, not complete, but we're building.

Benzos, Epidurals, and EHR, Oh My!

This is about benzodiazepines, epidurals and electronic health records. What a combination. I'm going to talk about benzodiazepines because it needs to be talked about, epidurals because it was asked to be talked about and electronic health records because no one talks about it. Benzodiazepines are wildly over prescribed. Many of our listeners are taking benzodiazepines, and they need to know about them. Remember Rule 4, know thy meds, know they categories, know the risk and benefits. Epidural management is a common procedure in the realm of pain management practice. This is one of the most common procedures performed throughout the world, and we're going to touch on explaining this procedure. This is part 1 of probably many parts. I'm going to touch a little bit on spinal anatomy, why you hurt, and move forward with a couple of comments on risk. Finally, I'm going to talk about electronic health records. What? Why is that important? Well it's important because it is a conversation. The medical record is a conversation among medical practitioners, and we are not the best communicators. Medicine is one of the last great enterprises to not understand the digital world. We're going into this slowly, as we should, and the electronic health record is a part of all of our lives. We are evolving together. I'm going to talk about the electronic health record more than once, from an individual who has built one, developed one, lectured and written about the electronic health record, and what it means to you. It is your information. It is a communication amongst professionals that can lead to a positive or negative outcome depending on how the information is conveyed, presented and communicated. Let's talk about this.

Pain vs. The World – Introduction to Opioids and Controlled Substances

In this episode I take a crack at a tough subject. Opioids and controlled substances are a fact of life. They are cost effective and a reasonable option to consider for many of the diagnoses we treat in the pain management arena. I start out by speaking of opioids and controlled substances in a general sense, and then work our way through terminology. Terminology is not an exciting topic, but necessary. Understanding key concepts of how we speak the language of pain helps us diminish misunderstanding, and barriers to care. Finally, I touch on a subject that's close to each of the members of the pain information network. People that take pain medicine are not necessarily addicts, and addicts are not bad people. Addiction is a disease, it needs to be treated. It is not a moral failure, but a brain that is gripped by a drug. Society should not judge these people as failures, but should reach out to them, and help them conquer their challenges. Addiction is an equal opportunity problem, and your reality states it best "there but for the grace of God go I."

They are used throughout the world, and are available through many sources, over-the-counter, and by prescription. A number of these medications have been associated with significant side effects, and this is discussed.
